Job Summary/Program Overview: TFV has become the agency of record for the Livingston Parish implementation of the My Community Cares initiative which is a diversion program for foster care. My Community Cares is a community-driven initiative that aims to fortify families and mitigate child abuse and neglect in the State of Louisiana. We will continue striving to build communities where every child and family are healthy, secure, and have equitable access to vital services and support. TFV is no stranger to this work as we have been doing it for years!

The Family Village is hiring a Case Manager to staff our Walker, LA location! The The TFV Case Manager will work closely with families, aiding in reducing the potential for initial or recurring involvement with the Department of Children and Family Services (DCFS). This role involves providing short-term case management for families that require extra support in identifying their needs and accessing existing resources.

Key Duties and Responsibilities:

Provides short-term case management for families requiring assistance in recognizing their needs and accessing available resources. Case management is delivered using the Solution Based Casework model or another established, evidence-based model used by our organization.

· Conducts preliminary intake and recurrent assessments with families while collecting, monitoring, and reporting related data using the systems and processes implemented in our organization.

· Connects individuals to services and supports within their community, encompassing: evaluating their present needs and identifying latent needs; problem-solving to address service and support accessibility issues; making and monitoring referrals to services; collaborating with agencies and community members to locate services and supports; advocating to overcome access barriers; providing short-term case management and follow-up services.

· Tracks and reports required qualitative and quantitative data to the Parish Director.
Preferred Skillset and Qualifications:​

Bachelor's degree in Social Work, or a related field, or equivalent work experience (3-5 years) in social services.

Certification or training in Trust-Based Relational Intervention® and Adverse Childhood Experiences (ACES) is a plus.

Demonstrable experience in partnering with families using a strengths-based approach.

Excellent written and verbal communication skills.

Ability to conduct needs assessments and provide appropriate resources and support.

Proven experience in managing cases, preferably within a social services context.

Knowledge of, and experience with, the Solution Based Casework model or other relevant, evidence-based models.

Ability to track and report qualitative and quantitative data accurately.

Familiarity with the Department of Children and Family Services (DCFS) and its procedures.

Proficiency in problem-solving, with an ability to overcome barriers to service accessibility.

Excellent collaboration skills, with a proven ability to work with other community agencies and members.

Ability to pass a background check with the Department of Children & Family Services

Cultural competency and the ability to work with diverse populations, BiLingual is a huge plus!

High level of discretion and professional approach to handling confidential information.

A passion for supporting families and communities, and a commitment to preventing child abuse and neglect.

The ideal candidate will be a compassionate, dedicated, and reliable professional with a strong commitment to providing support to families and communities. Experience in a similar role in the social services sector would be a significant advantage.
